Foster parents who are legal owners of their Home or property must submit proof of legal ownership( Photo Copy of Conveyance).Those who do not legally own their home but renting or Leasing from a landlord, must submit lease or rental agreement and are to notify the landlord of their intention to have foster children placed in the home. By signing this form, the landlord acknowledges that foster children will be living at the address given below.
