COMPLIANCE AGREEMENT ON MANDATORY AGENCY STANDARD
This agreement is by and between VARIETY CHILDREN & FAMILY SERVICES and:

- NAME OF CERTIFIED FOSTER PARENT
- I will provide a safe and holistic environment, teaching by example, an appropriate way of life. I understand that foster child (ren) place with me cannot stay overnight with any one, have other children stay overnight with foster children without notifying the placement agency
- I will not discriminate against any child that is placed in our home regardless of tribe, race, color, gender, religion, national origin, ancestry or sexual orientation.
- Monies received for my foster child shall be used according to the needs of the child.(Example Food , Clothing, Shelter, School, Medical).
- All personal valuables and clothing of the foster child will be kept safe for the child and remain the personal property of the child. I will keep an ongoing inventory of all property and clothing of my foster child and submit receipts to the office. These include gifts to the child while in placement in my home.
- I will attend all mandated meetings and training held as required for certification or re-certification by all government or non governmental placement agencies and Variety Children and Family services regulations.
- The nature of confidentiality in child care services has been explained to me by Variety Children and family Services and I understand and agree to all those expectations
- 7 I agree to discuss any and all information regarding the welfare of my foster child with Variety Foster family agency case manager and submit all progress reports or other incident reports when required.
- 8 All medical and dental emergencies including illnesses and injuries will be immediately reported to Variety administration. Receipts, notes ,letters etc from medical facilities will be turned in to the office on the same day.
- Follow all standardsregulations ,curfews,laws and consequences enforced by the government and Variety Children and family Services.
- I understand and have signed the statement acknowledging requirement to report all child abuse. I also understand I must attend a mandated child abuse reporting training session during my first year of certification.
- All visits to the foster child by natural family members must have prior approval by the assigned social worker.
- Right of access to the child as well as right of immediate removal from the foster home without prior notice remain with Variety Children and family Services.
- My home will be maintained in a reasonably clean manner and any family changes will be reported to Variety Children and Family Services Administration.
- I understand that direct contact with my foster child’s government or other placement agency is the function of Variety administration and I will not initiate such contact.
- I understand that any violation of these compliance agreements will subject me to an administrative action or a possible de-certification process which may include immediate removal of the child (ren) by Variety Children and family services.
- This signed agreement will remain in force for the entire period of certification.
